Required Documents
Once enrolled, you'll complete your child's registration through FinalForms. To ensure a smooth and efficient process, please have the following items ready to upload:
- Child's birth certificate OR passport
- Photo identification for a parent or legal guardian (driver's license, passport, state ID or military ID)
- Proof of residence
- If you own your home, current mortgage statement or deed AND a utility bill
- If you are building or purchasing a home, a purchase contract showing occupancy within 90 days of registration AND a utility bill once you have moved in
- If you are renting, a signed lease listing all occupants AND a utility bill
- If you are living in a residence owned by another person, this District Affidavit of Residency.pdf must be completed, signed and notarized by the homeowner and accompanied by a warranty deed or mortgage statement AND a utility bill
- Child's current immunization records
- Physical Examination Form
- Health History Form
- Dental Form
- Custody/Parenting Agreement (if applicable)
- School records or special education documentation (if applicable)
Academic Skills:
- Recognizes and writes their first name
- Recognizes some letters and letter sounds
- Counts objects and understands numbers to 10
- Listens to stories and participates in discussions
Social & Emotional Skills:
- Works and plays cooperatively with peers
- Demonstrates basic self-control and can manage emotions
- Shows curiosity and willingness to try new activities
- Can separate from a parent/caregiver with minimal difficulty
Communication Skills:
- Communicates needs, wants, and thoughts clearly
- Follows two- and three-step directions
Self-Help & Independence Skills:
- Uses the restroom independently
- Takes care of personal belongings
- Holds a pencil or crayon and draws/writes
- Holds scissors and is able to cut
